Make sure you have DOWNLOADED the CORRECT file. PRINT it. COMPLETE it. And RETURN it to the Officer Selection Team EITHER via FAX at (817)652-1088 or SCAN it into a PDF or DOC file format and EMAIL it as an ATTACHMENT to GySgt Gray.
